If you will be training at FIX Athletics with a personal or group training service, you must complete the following form. If you have just signed up for a Nutrition and Exercise plan and will be training at another facility you do not need to follow this step and can skip to the last step.

Please print out the form, complete, and sign. Then either scan and email the form to firstname.lastname@example.org or bring it with you to your first session or class.
IMPORTANT: If you do not have the form completed you will be unable to work out – no exceptions.
