If you will be training at FIX Athletics with a personal or group training service, you must complete the following form.

Please print out the form, complete, and sign. Then either scan and email the form to fixathletics@sasktel.net or bring it with you to your first session or class. 

IMPORTANT: If you do not have the form completed you will be unable to work out – no exceptions.

Note: If you have selected YES to any of the questions you are required to have your Doctor complete the PARmed-x form.
